Metuchen, N. J. (November 19, 2014) – Metuchen Public Library will host an Affordable Care Act Information Seminar and Enrollment on Saturday, December 20, 2014, 12:30 PM – 4:30 PM. Registration is required. Please call the Library at 732-632-8626. There will be a presentation about the Affordable Care Act followed by Enrollment provided by SRA International.
When you apply for coverage in the Health Insurance Marketplace, you will need to bring the following information:
- Social Security Numbers for all household members (and any immigration documents)
- Employer and income information or every member of your household (e.g. from pay stubs or W-2 forms – wage and tax statements)
- Current health insurance plan information for everyone in the household
